Medical Device Repair Terms & Acknowledgment

**MEDICAL DEVICE REPAIR TERMS & ACKNOWLEDGMENT**

By signing below, I acknowledge and agree to the following regarding medical or healthcare-related equipment serviced by Trilex Fix Repair Group LLC (“Trilex Fix”):

**1. Independent Electronics Repair Service**
Trilex Fix is an independent electronics repair provider and is not the manufacturer of the device, a medical provider, or a medical-device certification or calibration facility unless expressly stated in writing.

**2. Scope of Repair**
Trilex Fix's service is limited to the electronics repair, component replacement, troubleshooting, or other work specifically documented on the applicable repair ticket or invoice. Completion of a repair does not constitute certification that the entire device meets the manufacturer's specifications or applicable medical-device safety standards.

**3. No Certification for Patient or Clinical Use**
Any testing performed by Trilex Fix is intended to evaluate the functionality of the repair performed. Unless expressly documented otherwise, Trilex Fix does not perform or certify medical calibration, clinical accuracy, patient electrical-safety compliance, regulatory compliance, or suitability for diagnosis, monitoring, treatment, life-support, or other clinical use.

**4. Verification Required Before Patient Use**
I understand that the device should not be placed into clinical service or connected to a patient solely on the basis of Trilex Fix's repair or bench testing. Before patient or clinical use, I am responsible for having the device inspected, tested, calibrated, and/or certified as appropriate by the manufacturer, an authorized service provider, qualified biomedical equipment technician, or other appropriately qualified medical-equipment professional.

**5. Medical Decisions**
Trilex Fix does not provide medical advice and makes no representation regarding the clinical interpretation or accuracy of information produced by the device. Medical decisions must not be based on Trilex Fix's repair assessment or bench testing.

**6. Acknowledgment of Risk**
I understand that medical electronic equipment can malfunction and that a malfunction, inaccurate measurement, electrical fault, or other failure could potentially result in property damage, personal injury, serious injury, or death. I acknowledge the importance of appropriate post-repair safety and performance verification before the device is returned to clinical service.

**7. Limitation of Service**
I understand that Trilex Fix's completion and return of the device means only that the repair work documented on the repair ticket has been completed to the extent stated. It does not represent authorization, certification, or approval of the device for clinical or patient use.

By signing below, I confirm that I have read and understand this acknowledgment, have had the opportunity to ask questions, and accept responsibility for obtaining any required medical-device safety testing, calibration, certification, or professional inspection before the device is placed back into clinical or patient use.
